Gavriel Iddan
Gavriel Iddan (born October 1941 in Haifa) is the Israeli electro-optical engineer who invented capsule endoscopy, the pill-sized camera swallowed by a patient that photographs the small intestine from the inside and radios the pictures out. Marketed as the PillCam, his invention replaced a diagnostic dead zone: for decades the middle third of the human gut, the roughly 20-foot small intestine between the reach of an upper endoscope and a colonoscope, was effectively invisible to doctors without surgery. Iddan closed that gap by repurposing the guidance electronics of an air-to-air missile into a disposable capsule, and the company he co-founded to build it, Given Imaging, sold for $860 million to the medical device maker Covidien in 2014. It is now a Medtronic product used worldwide.
Who he is
Section titled “Who he is”Iddan spent his career as an engineer at Rafael Armament Development Authority, Israel’s state-owned defense R&D company, working on the seeker, the electro-optical “eye” that locks a guided missile onto its target. It is exactly the kind of defense-sector engineering post that has, across Israeli tech history, produced founders in unrelated civilian fields once conscription-era or career-long work in the IDF and its supporting industries on miniaturized sensors, optics and signal transmission is turned toward a private problem, the same pattern behind Simcha Blass’s drip irrigation and Dov Moran’s USB flash drive. Iddan’s private problem arrived during a sabbatical year in Boston in 1981, when a neighbor, Israeli gastroenterologist Eitan Scapa, was suffering from undiagnosed abdominal pain and challenged him: could the same seeker technology that finds a target from a missile nose cone be built small enough to travel through a human gut and photograph it from inside?
What he built
Section titled “What he built”The problem was not simply making a tiny camera. It was making a tiny camera that could light, image, compress and wirelessly transmit thousands of frames from inside a moving, opaque, fluid-filled tube, running on a battery small enough to swallow, without wires, without a doctor steering it, and without harming the patient. Early charge-coupled device (CCD) imaging chips, the standard camera sensor of the era, drew too much power to survive more than about ten minutes inside the body, far short of the six-to-eight-hour transit time through the small intestine. Iddan’s breakthrough came when he recognized that CMOS image sensors, then an emerging alternative, consumed roughly one percent the power of a CCD chip for a comparable image, solving the battery problem that had stalled every earlier attempt at a swallowable camera. He filed the foundational patent, “In vivo video camera system,” in January 1995 with Rafael colleague Doron Sturlesi; it was granted in February 1997.
How it reached patients
Section titled “How it reached patients”Iddan co-founded Given Imaging in 1998 with Gavriel Meron to commercialize the capsule outside Rafael. The first working capsule, an ellipsoid barely larger than a large vitamin pill containing a lens, LED lights, a CMOS imager, a transmitter and a battery, was swallowed by a human volunteer for the first time in October 1999, part of a trial led by British gastroenterologist Paul Swain. Given Imaging’s device received CE marking in Europe in May 2001 and US FDA clearance that August for imaging the small intestine, the diagnostic zone conventional endoscopy could not reach. Within roughly fifteen years, capsule endoscopy generated over 2,500 peer-reviewed clinical publications documenting its use, and the PillCam line expanded beyond the small bowel to the esophagus and colon, becoming, in Medtronic’s own description at the time of acquisition, “the most widely used, patient-friendly tool for directly visualizing the small bowel” in a multibillion-dollar global gastrointestinal device market.
Who ended up owning it, and why that doesn’t rewrite who invented it
Section titled “Who ended up owning it, and why that doesn’t rewrite who invented it”Given Imaging was acquired by the Irish-domiciled medical device company Covidien in a deal that closed 27 February 2014, at $30 a share, worth approximately $860 million net of cash. Covidien was itself bought by Medtronic in 2015, so the PillCam, an Israeli invention built on Israeli defense-sector engineering, ended up owned by an American multinational. Iddan has said publicly that he regrets the company leaving Israeli hands rather than staying domestically owned and grown, a common tension in Israeli tech: the country’s research base produces outsized breakthroughs, but the exits that reward their inventors often move the resulting company, its manufacturing and its long-term profits abroad. None of that touches who actually built it. The capsule itself, and the patents behind it, trace to a Rafael engineer’s missile-seeker know-how and a doctor neighbor’s stomach pain, not to any foreign R&D budget.
Why it matters
Section titled “Why it matters”Before capsule endoscopy, diagnosing bleeding, tumors, Crohn’s disease activity or polyps deep in the small intestine often meant either inconclusive imaging or an invasive procedure: push enteroscopy, intraoperative endoscopy, or open surgery, each carrying real risk and recovery time for a diagnostic look. The PillCam turned that into an outpatient procedure: swallow a pill, wear a small recorder for about eight hours, go home. Iddan received the European Patent Office’s European Inventor Award nomination in 2011, fourteen years after his original patent was granted, recognition of an invention that a country of roughly nine million people, built and defended under permanent security pressure, produced by pointing a military sensor technology at an ordinary human problem instead of at a target. It is the same chutzpah that let a missile engineer tell a gastroenterologist yes, a targeting seeker can be shrunk to fit inside a pill.
